Practical Solutions to Prevent and Prepare for Severe Hypoglycemia
1
Do you review the occurrence and risk for hypoglycemia in patients with diabetes at every encounter?
2
Are your patients with diabetes prepared for an episode of severe hypoglycemia?
3
Do you know whether your at-risk patients have an active prescription for glucagon?

These tools and educational resources were compiled from a CME/CE initiative and Quality Improvement (QI) program sponsored by Purdue University College of Pharmacy, in partnership with the Academy for Continued Healthcare Learning (ACHL), and supported by an educational grant from Lilly.

these tools are essential for your practice:

The program we’ve developed has a lot of different aspects so that we really can meet the needs of healthcare providers so that at the end of the day they feel much more comfortable about hypoglycemia. These tools are important to me because you may not always have an opportunity to develop your own tools or find tools that meet the needs of your clinical practice.

I think initiatives such as this one are so important to help us as clinicians keep track of all of the things that we need to keep track of when dealing with a patient, but in particular with our patients with diabetes – the notion of preparing them in case an episode of severe hypoglycemia occurs and making sure they have the tools to treat it if an episode happens.
